# Liquid Paraffin
## Overview
Liquid paraffin is a highly refined mineral oil used as a laxative and emollient. It acts as a lubricant, softening stool and coating the intestinal lining to ease passage.
## Primary Indications
* Constipation (short-term relief)
* Prevention of fecal impaction
## Adult Dosing
* **Laxative:** 15 mL to 30 mL once daily, usually at bedtime. Maximum dose not explicitly defined but typically not exceeding 45 mL per day.
## Pediatric Dosing
* **Constipation:**
* 1 to 5 years: 5 mL once daily.
* 6 to 12 years: 10 mL once daily.
* Over 12 years: 15 mL once daily.
* Dosing may be adjusted based on response and local protocol. Doses are usually given at bedtime.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment.
## Contraindications
* Appendicitis, intestinal obstruction, inflammatory bowel disease (Crohn's disease, ulcerative colitis), abdominal pain of unknown origin.
* Individuals with difficulty swallowing due to risk of aspiration.

* Anal leakage/soiling
* Abdominal cramping or discomfort

## Key Drug Interactions
* **Mineral Oil:** Concurrent use with other mineral oil-containing products can increase the risk of adverse effects.
* **Fat-soluble Vitamins (A, D, E, K):** Absorption may be reduced with chronic use.
* **Docusate:** Co-administration may enhance stool softening.
## Monitoring
* Bowel movements (frequency and consistency)
* Signs of anal leakage or irritation.
* For chronic use, monitor for signs of vitamin malabsorption.
## Clinical Pearls
* Administer at bedtime to minimize daytime leakage.
* Avoid prolonged use (generally recommended for no more than 1 week) to prevent dependence and fat-soluble vitamin malabsorption.
* Aspiration is a serious risk, especially in children and the elderly, and can lead to lipoid pneumonia. Use with caution in individuals with impaired gag reflexes or reflux.
* Should not be taken immediately before lying down.
