# Liquid Paraffin
## Overview
Liquid paraffin is a mineral oil used as an emollient and laxative. It lubricates the intestinal tract, softening stool and facilitating bowel movements.
## Primary Indications
* Constipation
## Adult Dosing
* **Laxative:** 10-30 mL once daily, usually at bedtime. Doses can be adjusted based on response. Do not exceed 30 mL per day unless otherwise directed by a healthcare provider.
## Pediatric Dosing
* **Laxative:**
* 1-5 years: 5 mL once daily.
* 6-12 years: 10 mL once daily.
* Over 12 years: 10-30 mL once daily.
* **Note:** Dosing in children should generally be for short-term use. Consult a pediatrician for prolonged use or if symptoms persist.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment, as systemic absorption is minimal.
## Contraindications
* Hypersensitivity to liquid paraffin.
* Patients with intestinal obstruction, appendicitis, or acute abdominal pain.
* Prolonged use in children should be avoided due to potential interference with fat-soluble vitamin absorption (A, D, E, K).
## Adverse Effects
* Diarrhea, abdominal cramping, nausea, vomiting.
* Anal leakage or soiling (especially with higher doses or in debilitated patients).
* Rarely: lipid pneumonia if aspirated (particularly in infants, elderly, or those with impaired swallowing).
* Long-term use may interfere with the absorption of fat-soluble vitamins.
## Key Drug Interactions
* **Fat-soluble vitamins (A, D, E, K):** Liquid paraffin may decrease their absorption with chronic use. Separation of administration is recommended.
* **Other laxatives:** Concurrent use can lead to excessive fluid loss and electrolyte imbalance.
## Monitoring
* Bowel movements and stool consistency.
* Signs of abdominal discomfort or cramping.
* For chronic use in children: monitor for signs of vitamin deficiency.
## Clinical Pearls
* Administer undiluted or mixed with water or juice.
* Avoid administration immediately before lying down to minimize the risk of aspiration and lipid pneumonia.
* Inform patients that anal leakage may occur.
* Long-term daily use is generally discouraged due to potential vitamin malabsorption and dependence.
