# Liquid Paraffin
## Overview
Liquid paraffin is a mineral oil used as a laxative. It lubricates the stool and intestinal walls, facilitating easier passage.
## Primary Indications
* Constipation
## Adult Dosing
* **Oral:** 10 mL to 30 mL once daily, or as needed. Maximum dose is typically 45 mL per day. Dosing may vary based on product formulation and local protocol.
## Pediatric Dosing
* **Children 6 to 12 years:** 5 mL to 15 mL once daily.
* **Children 1 to 5 years:** 2.5 mL to 5 mL once daily.
* **Infants under 1 year:** Generally not recommended. If used, it should be under strict medical supervision and at very low doses (e.g., 1-2 mL once daily). Consult pediatrician for specific dosing.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment.
## Contraindications
* Hypersensitivity to liquid paraffin.
* Appendicitis or undiagnosed abdominal pain.
* Intestinal obstruction or perforation.
* Difficulty swallowing (risk of aspiration).
## Adverse Effects
* Abdominal cramping
* Nausea
* Vomiting
* Diarrhea
* Anal leakage
* Lipoid pneumonia (if aspirated)
* Decreased absorption of fat-soluble vitamins (A, D, E, K) with chronic use.
## Key Drug Interactions
* **Fat-soluble vitamins (A, D, E, K):** Absorption may be decreased with concurrent use, especially with chronic administration. Separate administration by at least 2 hours.
* **Other laxatives:** Concurrent use may increase the risk of diarrhea and electrolyte imbalance.
## Monitoring
* Bowel movements (frequency, consistency)
* Signs of dehydration or electrolyte imbalance if diarrhea occurs.
* For chronic use, monitor for potential vitamin deficiencies.
## Clinical Pearls
* Administer undiluted, preferably on an empty stomach or at bedtime.
* Avoid taking within 2 hours of other medications to minimize potential absorption interference.
* Risk of aspiration is significant, especially in infants, the elderly, or those with swallowing difficulties. Do not administer while lying down.
* Chronic use can lead to dependence and vitamin malabsorption; it is generally intended for short-term relief.
* Anal leakage can be a nuisance; advise patients to use sanitary pads.
