# Liquid Paraffin
## Overview
Liquid paraffin is a highly refined mineral oil used as a lubricant and emollient. It acts as a laxative by softening stool and easing passage.
## Primary Indications
* Constipation
## Adult Dosing
* **Laxative:** 15 mL to 45 mL orally once daily or in divided doses. Higher doses may be used under medical supervision for more severe constipation.
## Pediatric Dosing
* **Constipation:** Dosing varies by age and weight. Generally:
* 1 to 5 years: 5 mL to 10 mL orally once daily.
* 6 to 12 years: 10 mL to 15 mL orally once daily.
* Over 12 years: As per adult dosing.

## Dose Adjustments
No dose adjustments are typically required for hepatic or renal impairment.
## Contraindications
* Appendicitis or undiagnosed abdominal pain.
* Intestinal obstruction.

* Difficulty swallowing (risk of aspiration).
## Adverse Effects
* **Common:** Abdominal cramping, bloating, diarrhea, anal leakage.
* **Less Common/Serious:**
* Lipoid pneumonia (if aspirated, particularly in the elderly or those with impaired swallowing).
* Malabsorption of fat-soluble vitamins (with chronic, high-dose use).
* Pruritus ani (anal itching).
## Key Drug Interactions
* **Mineral Oil:** May reduce the absorption of fat-soluble vitamins (A, D, E, K) and certain medications, including digoxin, anticoagulants, and oral contraceptives, when taken concurrently. Separate administration by at least 2 hours.
## Monitoring
* Monitor for bowel movements and stool consistency.
* Assess for signs of anal leakage or perianal irritation.
* In patients with risk factors for aspiration, monitor for respiratory symptoms.
## Clinical Pearls
* Administer undiluted or mixed with water or juice.
* Avoid administration immediately before lying down to reduce the risk of aspiration.
* Long-term, habitual use can lead to dependence and interfere with nutrient absorption.
* Caution in elderly patients and those with swallowing difficulties due to aspiration risk.
