# Liquid Paraffin
## Overview
Liquid paraffin is a highly refined mineral oil used as an emollient and laxative. It lubricates the intestinal tract, softening stool and promoting bowel movements.
## Primary Indications
* Constipation
## Adult Dosing
* **Laxative:** 10-30 mL once daily, typically at bedtime. Doses can be adjusted based on patient response.
## Pediatric Dosing
* Dosing in children is not well established and should be based on specific clinical guidelines or expert consultation. Generally, lower doses are used for younger children, but caution is advised due to the risk of aspiration and anal leakage. Consult product labeling or a pediatrician for specific recommendations.
## Dose Adjustments
* No specific dose adjustments are generally required for hepatic or renal impairment.
## Contraindications
* Known hypersensitivity to liquid paraffin.
* Patients with difficulty swallowing due to the risk of aspiration, which can lead to lipoid pneumonia.
* Should be used with caution in the elderly and debilitated due to increased risk of anal leakage and aspiration.
* Avoid in patients with inflammatory bowel disease or intestinal obstruction.
## Adverse Effects
* Anal leakage (especially with higher doses or if taken upright).
* Abdominal cramping or discomfort.
* Nausea.
* Risk of lipoid pneumonia if aspirated into the lungs.
* Interference with the absorption of fat-soluble vitamins (A, D, E, K) with chronic use.
## Key Drug Interactions
* May decrease the absorption of fat-soluble vitamins (A, D, E, K).
* May decrease the absorption of other orally administered medications, particularly those that are lipid-soluble. Administer other medications at least 2 hours apart from liquid paraffin.
## Monitoring
* Bowel movement frequency and consistency.
* Signs of anal leakage or soiling.
* For long-term users, monitor for signs of vitamin deficiency.
## Clinical Pearls
* Administer at bedtime to minimize the risk of anal leakage.
* Advise patients to take it in an upright position to reduce aspiration risk.
* Chronic use may lead to dependence on laxatives and vitamin deficiencies.
