# Liquid Paraffin
## Overview
Liquid paraffin is a mineral oil used as a lubricant laxative. It coats the fecal mass, easing its passage.
## Primary Indications
* Constipation
## Adult Dosing
* **Oral:** 10 mL to 30 mL once daily, usually at bedtime. Dosing can be adjusted based on response. Maximum daily dose is generally not specified but should be used cautiously.
## Pediatric Dosing
* **Oral (6-12 years):** 5 mL to 15 mL once daily, usually at bedtime.
* **Oral (1-5 years):** 2.5 mL to 5 mL once daily, usually at bedtime.
* **Oral (<1 year):** Not generally recommended. If used, dosing should be determined by a healthcare provider and is typically very low (e.g., 1-2.5 mL once daily).
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment.
## Contraindications
* Intestinal obstruction
* Abdominal pain of unknown cause
* Nausea or vomiting
* Hypersensitivity to liquid paraffin
## Adverse Effects
* Gastrointestinal: Abdominal cramping, diarrhea, nausea.
* Anal leakage: Can occur, especially with higher doses.
* Lipoid pneumonia: A risk if aspirated, particularly in infants and the elderly.
* Fat-soluble vitamin malabsorption: With chronic, high-dose use, may interfere with the absorption of vitamins A, D, E, and K.
## Key Drug Interactions
* Concurrent use with other laxatives may increase the risk of diarrhea and electrolyte imbalance.
* May reduce the absorption of orally administered drugs, particularly fat-soluble vitamins. Consider administration at a different time than other medications.
## Monitoring
* Bowel movement frequency and consistency.
* Signs of dehydration or electrolyte imbalance with prolonged or high-dose use.
## Clinical Pearls
* Administer undiluted or mixed with water.
* Avoid administration immediately before lying down to minimize the risk of aspiration and subsequent lipoid pneumonia.
* Long-term daily use should be discouraged due to potential for dependency and vitamin malabsorption.
* Ensure adequate hydration when using laxatives.
