## Liquid Paraffin
### Overview
Liquid paraffin is a mineral oil used as a laxative. It works by softening the stool and lubricating the intestinal tract, making bowel movements easier.
### Primary Indications
* Constipation
### Adult Dosing
* **Oral:** 10 mL to 30 mL once daily, typically at bedtime.
* Maximum dose not explicitly established but generally kept to the lower end of the range for chronic use.
### Pediatric Dosing
* **Oral:**
* 1 to 5 years: 5 mL once daily at bedtime.
* 6 to 12 years: 5 mL to 10 mL once daily at bedtime.
* Over 12 years: Same as adult dosing.
* Dosing for infants under 1 year is generally not recommended and should be under strict medical supervision.
### Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment.
### Contraindications
* Appendicitis or symptoms of acute abdominal conditions (e.g., nausea, vomiting, abdominal pain).
* Intestinal obstruction.
* Inflammatory bowel disease (e.g., Crohn's disease, ulcerative colitis) due to risk of exacerbation.
* Difficulty swallowing (risk of aspiration).
### Adverse Effects
* **Common:** Abdominal cramping, bloating, diarrhea.
* **Less Common:** Nausea, vomiting.
* **Serious:**
* **Lipoid pneumonia:** If aspirated, particularly in individuals with impaired gag reflexes or in the supine position during administration.
* **Anal leakage:** May occur, especially with higher doses.
* **Reduced absorption of fat-soluble vitamins (A, D, E, K):** With chronic, long-term use.
### Key Drug Interactions
* **Mineral oil can decrease the absorption of fat-soluble vitamins (A, D, E, K) and other lipid-soluble drugs.** Separate administration by at least 2 hours.
* **Concurrent use with docusate sodium:** May increase absorption of mineral oil, increasing the risk of adverse effects.
### Monitoring
* Monitor for effectiveness (bowel movements).
* Assess for abdominal pain, distention, or signs of obstruction.
* Monitor for anal leakage and potential for skin irritation.
* Consider monitoring for vitamin deficiencies with chronic long-term use.
### Clinical Pearls
* Administer at bedtime to minimize daytime inconvenience from anal leakage.
* Advise patients to remain in an upright position for at least 30 minutes after taking liquid paraffin to reduce aspiration risk.
* Avoid in patients with difficulty swallowing or those at risk of aspiration.
* Not recommended for long-term daily use due to potential for vitamin malabsorption and dependency.
* If symptoms persist or worsen after a few days of use, patients should seek medical advice.
