# Liquid Paraffin
## Overview
Liquid paraffin is a mineral oil used as a laxative and emollient. It works by lubricating the intestinal walls, softening stool, and preventing water absorption from the feces.
## Primary Indications
* Constipation
* Skin conditions (e.g., dryness, eczema) as an emollient
## Adult Dosing
* **Laxative:** 10-30 mL once daily, typically at bedtime.
* **Emollient:** Apply thinly to affected skin areas as needed.
## Pediatric Dosing
* **Laxative:**
* Children 1-5 years: 5 mL once daily.
* Children 6-12 years: 10 mL once daily.
* Children over 12 years: Dosing as per adult.
* Use in children under 1 year is generally not recommended without medical supervision due to the risk of aspiration.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment.
## Contraindications
* Hypersensitivity to liquid paraffin.
* Appendicitis or acute abdominal pain.
* Intestinal obstruction.
* Inability to swallow without risk of aspiration (particularly in infants and the elderly).
## Adverse Effects
* Anal leakage (especially with higher doses).
* Abdominal cramping.
* Nausea.
* Lipoid pneumonia (rare, associated with aspiration).
* Reduced absorption of fat-soluble vitamins (A, D, E, K) with chronic, high-dose use.
## Key Drug Interactions
* **Mineral oil can reduce the absorption of:** fat-soluble vitamins (A, D, E, K), anticoagulants (e.g., warfarin), and some cardiac glycosides. Dosing should be separated.
## Monitoring
* Monitor for bowel movements and stool consistency.
* Assess for anal leakage or signs of aspiration.
* For long-term use, consider monitoring for vitamin deficiencies, especially in vulnerable populations.
## Clinical Pearls
* Administer at bedtime to minimize daytime leakage.
* Avoid prolonged use (generally no more than 1-2 weeks) due to potential for dependency and malabsorption.
* Warn patients about the risk of slippery surfaces due to anal leakage.
* Ensure adequate hydration when used as a laxative.
* For emollient use, apply to damp skin for better absorption.
