# Liquid Paraffin
## Overview
Liquid paraffin is a highly refined mineral oil used as an emollient and laxative. It works by lubricating the intestinal tract, softening the stool, and promoting bowel movements.
## Primary Indications
* Constipation
## Adult Dosing
* **Laxative:** 10 mL to 30 mL once daily, usually at bedtime. The dose may be adjusted based on response.
## Pediatric Dosing
* **Laxative:** Dosing varies by age and is typically determined by local protocols and prescriber preference.
* Children 1-5 years: 5 mL once daily.
* Children 6-12 years: 10 mL once daily.
* Consult with a pediatrician or refer to pediatric formularies for specific age-based dosing guidelines.
## Dose Adjustments
* No specific dose adjustments are typically required for renal or hepatic impairment.
## Contraindications
* Appendicitis or acute abdominal pain.
* Intestinal obstruction.
* Unexplained rectal bleeding.
* Fecal impaction.
* Difficulty swallowing (risk of aspiration).
## Adverse Effects
* Anal leakage (especially with higher doses).
* Abdominal cramps or discomfort.
* Nausea.
* Lipoid pneumonia (rare, risk associated with chronic aspiration).
* Reduced absorption of fat-soluble vitamins (A, D, E, K) with chronic, long-term use.
## Key Drug Interactions
* **Fat-soluble vitamins (A, D, E, K):** Long-term use may reduce absorption. Consider separate administration times or vitamin supplementation if clinically indicated.
* **Docusate:** Concurrent use is generally not recommended as it may increase the absorption of mineral oil.
## Monitoring
* Bowel movements (frequency, consistency).
* Signs of abdominal discomfort or cramping.
* Assess for anal leakage.
* Monitor for signs of aspiration, especially in elderly or debilitated patients.
## Clinical Pearls
* Administer undiluted and do not lie down immediately after taking.
* Warn patients about the potential for anal leakage.
* Avoid prolonged or excessive use due to potential vitamin malabsorption and risk of lipoid pneumonia.
* It is not absorbed systemically and is generally considered safe for short-term use.
