# Liquid Paraffin
## Overview
Liquid paraffin is a highly refined mineral oil used as a lubricant laxative. It coats the stool, making it softer and easier to pass. It is not absorbed by the body.
## Primary Indications
Constipation.
## Adult Dosing
* **Oral:** 10-30 mL once daily, usually at bedtime.
* **Rectal (as an enema):** 100-120 mL per rectum.
## Pediatric Dosing
* **Oral:** Dosing varies by age and is generally lower than adult doses. For example, 1-5 years: 5 mL once daily. 6-12 years: 10 mL once daily. Specific pediatric dosing should be guided by local protocols or a pediatrician.
* **Rectal:** Not typically recommended for routine use in young children without medical supervision due to risk of complications.
## Dose Adjustments
No dose adjustments are typically needed based on renal or hepatic impairment as it is not absorbed.
## Contraindications
* Appendicitis or undiagnosed abdominal pain.
* Intestinal obstruction.
* Fecal impaction.
* Nausea or vomiting.
* Hypersensitivity to liquid paraffin.
## Adverse Effects
* Anal leakage (most common), soiling of underwear.
* Abdominal cramping, nausea, vomiting, diarrhea (especially with excessive doses).
* Lipoid pneumonia (rare, associated with chronic aspiration, particularly in elderly or debilitated patients).
* Vitamin malabsorption (long-term, high-dose use).
## Key Drug Interactions
* **Mineral oil can decrease the absorption of fat-soluble vitamins (A, D, E, K) and certain medications absorbed orally** (e.g., digoxin, anticoagulants, oral contraceptives). Administer at least 2 hours apart from other oral medications and vitamin supplements.
## Monitoring
* Bowel movements and frequency.
* Signs of abdominal distension or obstruction.
* Signs of lipid pneumonia (e.g., cough, fever, shortness of breath) in patients at risk.
## Clinical Pearls
* Administer at bedtime to minimize anal leakage during the day.
* Advise patients to use sanitary pads or protective underwear to manage potential anal leakage.
* Long-term or excessive use should be avoided due to potential for vitamin deficiency and lipid pneumonia.
* Not intended for short-term relief of constipation; longer-acting agents may be preferred.
