# Liquid Paraffin
## Overview
Liquid paraffin is a mineral oil used as an emollient and laxative.
## Primary Indications
* Lubricant for constipation.
* Topical emollient for dry skin conditions.
## Adult Dosing
### Constipation:
* **Oral:** 10-30 mL once daily, typically at bedtime.
### Topical Emollient:
* Apply thinly and as needed.
## Pediatric Dosing
### Constipation:
* **Oral:**
* **1-5 years:** 5 mL once daily.
* **6-12 years:** 10 mL once daily.
* **>12 years:** 10-20 mL once daily.
* *Note: Dosing in children under 1 year should be at the discretion of a healthcare professional due to increased risk of aspiration.*
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment.
## Contraindications
* Hypersensitivity to liquid paraffin.
* Patients with difficulty swallowing due to risk of aspiration and lipoid pneumonia.
* Inflammatory bowel disease (e.g., Crohn's disease, ulcerative colitis).
* Appendicitis or symptoms of acute abdominal pain.
## Adverse Effects
* **Gastrointestinal:** Abdominal cramps, diarrhea, nausea, vomiting, anal leakage (especially with higher doses or inadequate fluid intake).
* **Aspiration:** Lipoid pneumonia (rare but serious, particularly if taken orally in a recumbent position or by individuals with impaired gag reflex).
* **Topical:** Skin irritation, rash.
## Key Drug Interactions
* **Absorption of Fat-Soluble Vitamins:** May decrease the absorption of vitamins A, D, E, and K. Consider separation of administration or vitamin supplementation.
* **Docusate:** Concurrent use may increase the risk of anal leakage.
## Monitoring
* Assess for bowel movement frequency and consistency.
* Monitor for signs of abdominal discomfort or leakage.
* Observe for signs of aspiration or lipoid pneumonia if risk factors are present.
## Clinical Pearls
* Administer orally between meals or at bedtime to minimize the risk of aspiration.
* Advise patients to drink a full glass of water with each oral dose to help prevent constipation and aid passage.
* Avoid use in patients who are frail, elderly, or have a compromised gag reflex.
* Long-term use of oral liquid paraffin is generally not recommended due to potential for vitamin malabsorption and anal leakage.
