# Liquid Paraffin
## Overview
Liquid paraffin is a mineral oil used as a lubricant laxative. It coats the fecal mass, softening it and easing passage.
## Primary Indications
Constipation.
## Adult Dosing
* **Oral:** 10 mL to 30 mL once daily or in divided doses. Dosing is typically initiated at the lower end and titrated as needed. Maximum dose is generally not explicitly defined but should be based on efficacy and tolerance.
## Pediatric Dosing
* **Oral:** Dosing varies significantly by age and formulation. Specific product labeling should be consulted.
* **Children 6-12 years:** Typically 5 mL to 15 mL once daily.
* **Children 1-5 years:** Typically 2.5 mL to 5 mL once daily.
* **Infants < 1 year:** Not generally recommended due to risk of aspiration and potential for adverse effects. Consult pediatrician.
## Dose Adjustments
* No specific dose adjustments are typically required for hepatic or renal impairment.
## Contraindications
* Hypersensitivity to liquid paraffin.
* Intestinal obstruction, acute abdominal conditions, appendicitis, inflammatory bowel disease, fecal impaction, or rectal bleeding of unknown origin.
* Use with caution in patients with dysphagia or impaired gag reflex due to risk of aspiration.
## Adverse Effects
* Most common: Anal leakage, oily stools, abdominal cramps, bloating, nausea.
* Long-term, high-dose use may lead to vitamin malabsorption (especially fat-soluble vitamins A, D, E, K) and pneumonitis if aspirated.
## Key Drug Interactions
* Can reduce the absorption of fat-soluble vitamins and other orally administered medications by decreasing their absorption. Separate administration by at least 2 hours.
* Concurrent use with docusate may increase the risk of adverse effects.
## Monitoring
* Bowel movement frequency and consistency.
* Signs of anal leakage or perianal irritation.
* Monitor for signs of vitamin deficiency with chronic use.
* Assess for abdominal pain or distension.
## Clinical Pearls
* Best taken at bedtime to minimize anal leakage during waking hours.
* Avoid administration immediately before lying down due to aspiration risk.
* Not for long-term use without medical supervision.
* Ensure adequate fluid intake when using laxatives.
