# Liquid Paraffin
## Overview
Liquid paraffin is a mineral oil used as a laxative. It works by lubricating the stool and the intestinal walls, making defecation easier.
## Primary Indications
* **Constipation:** For the relief of occasional constipation.
## Adult Dosing
* **Oral:** 10 mL to 30 mL once daily, typically at bedtime. Do not exceed 30 mL in a 24-hour period.
## Pediatric Dosing
* **Oral (6-12 years):** 5 mL once daily at bedtime.
* **Oral (under 6 years):** Not generally recommended. Consult a pediatrician for appropriate management.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment, as absorption is minimal.
## Contraindications
* **Abdominal pain, nausea, vomiting, or other symptoms of appendicitis or undiagnosed abdominal pain:** Use may mask serious underlying conditions.
* **Patients with difficulty swallowing:** Risk of aspiration and lipoid pneumonia.
* **Hypersensitivity to liquid paraffin.**
## Adverse Effects
* **Gastrointestinal:** Anal leakage, abdominal cramping, diarrhea.
* **Aspiration:** Can lead to lipoid pneumonia, especially in the elderly or those with impaired gag reflexes.
* **Malabsorption:** Long-term, high-dose use may interfere with the absorption of fat-soluble vitamins (A, D, E, K).
## Key Drug Interactions
* **Fat-soluble vitamins:** May decrease absorption.
* **Steroids:** May decrease absorption.
* **Certain medications taken orally:** May decrease absorption due to reduced transit time or coating of the intestinal mucosa. Separate administration by at least 2 hours.
## Monitoring
* Monitor for relief of constipation.
* Assess for signs of anal leakage or abdominal cramping.
* Consider monitoring for fat-soluble vitamin deficiencies with prolonged, regular use.
## Clinical Pearls
* Administer at bedtime to reduce the likelihood of anal leakage during the day.
* Avoid administering in a supine position or to individuals with impaired swallowing to minimize the risk of aspiration.
* Not intended for chronic use; regular bowel habits should be encouraged through diet and lifestyle modifications.
* The risk of lipoid pneumonia is a serious concern, particularly in vulnerable populations.
