# Liquid Paraffin
## Overview
Liquid paraffin is a mineral oil used as a laxative. It lubricates the intestinal walls, softening stool and facilitating bowel movements. It is not absorbed by the body.
## Primary Indications
* Short-term treatment of constipation.
## Adult Dosing
* **Oral:** 10 mL to 40 mL once daily, typically at bedtime. The specific dose should be individualized to achieve a soft bowel movement.
## Pediatric Dosing
* **Oral (Under 12 years):** Generally not recommended due to risk of aspiration. If used, it should be under strict medical supervision and typically at a reduced dose (e.g., 5 mL once daily for younger children), with careful administration to minimize aspiration risk. Consult a pediatrician for specific dosing.
## Dose Adjustments
* No dose adjustments are typically required for renal or hepatic impairment, as it is not absorbed systemically.
## Contraindications
* Hypersensitivity to liquid paraffin.
* Patients with difficulty swallowing or a risk of aspiration.
* Appendicitis or other acute abdominal conditions.
* Intestinal obstruction.
* Use with caution in bedridden patients or those with impaired gag reflex.
## Adverse Effects
* Diarrhea, abdominal cramping, nausea, vomiting.
* Anal leakage (especially with higher doses).
* Lipoid pneumonia (if aspirated into the lungs, particularly with prolonged use or in susceptible individuals).
* Malabsorption of fat-soluble vitamins (A, D, E, K) with prolonged or excessive use.
## Key Drug Interactions
* **Fat-soluble vitamins:** Prolonged use may decrease absorption.
* **Anticoagulants:** Potential for decreased absorption and efficacy, although clinically significant interactions are rare.
## Monitoring
* Monitor for bowel movement frequency and consistency.
* Assess for signs of abdominal pain, cramping, or anal leakage.
* Monitor for signs of aspiration, especially in vulnerable populations.
## Clinical Pearls
* Administer undiluted.
* Avoid taking immediately before lying down due to the risk of aspiration.
* Do not use for prolonged periods without medical advice.
* If constipation persists, further investigation may be warranted.
