# Liquid Paraffin
## Overview
Liquid paraffin is a mineral oil used as an emollient and laxative. It lubricates the intestinal tract, softening stool and easing defecation.
## Primary Indications
* **Constipation:** Relief of occasional constipation.
* **Stool Softener:** To facilitate bowel movements in conditions where straining should be avoided (e.g., post-surgery, hemorrhoids).
## Adult Dosing
* **Laxative:** 10-45 mL once daily, typically at bedtime.
* **Stool Softener:** 10-15 mL once daily.
## Pediatric Dosing
* **Constipation (Children 6-12 years):** 5-20 mL once daily.
* **Constipation (Children 1-5 years):** 5-10 mL once daily.
* **Note:** Use in infants under 1 year is generally not recommended without medical supervision due to the risk of aspiration and potential adverse effects.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment. Dosing is primarily based on patient age and clinical indication.
## Contraindications
* Patients with difficulty swallowing or a history of aspiration.
* Appendicitis or undiagnosed abdominal pain.
* Intestinal obstruction.
* Hypersensitivity to liquid paraffin.
## Adverse Effects
* **Common:** Abdominal cramping, bloating, diarrhea, anal leakage.
* **Serious/Rare:** Lipoid pneumonia (if aspirated), vitamin malabsorption (with chronic use).
## Key Drug Interactions
* **Fat-soluble vitamins (A, D, E, K):** Long-term, regular use may decrease absorption. Consider separating administration by at least 2 hours.
* **Other laxatives:** Concurrent use may increase the risk of dehydration and electrolyte imbalance.
## Monitoring
* Effectiveness: Monitor for bowel movement frequency and consistency.
* Adverse effects: Assess for abdominal pain, cramping, anal leakage, and signs of aspiration.
* Long-term use: Consider monitoring for potential vitamin deficiencies if used chronically.
## Clinical Pearls
* Administer at bedtime to minimize disruption and potential anal leakage during the day.
* Avoid administration immediately before lying down due to aspiration risk.
* Not intended for chronic use; encourage lifestyle modifications for long-term management of constipation.
