# Liquid Paraffin
## Overview
Liquid paraffin is a mineral oil used as an emollient and laxative. It works by coating the stool, making it softer and easier to pass, and by stimulating bowel movements.
## Primary Indications
* **Constipation:** Treatment of occasional or short-term constipation.
* **Lubricant:** Used topically as an emollient for dry skin.
## Adult Dosing
* **Laxative:** 15-45 mL once daily, typically at bedtime.
* **Topical Emollient:** Apply to affected skin as needed.
## Pediatric Dosing
* **Laxative:** Dosing is highly variable and depends on age and weight. Consult specific pediatric guidelines or a pediatrician. Generally, lower doses are used for younger children. For example:
* 1-5 years: 5-10 mL once daily.
* 6-12 years: 10-15 mL once daily.
* Over 12 years: May use adult dosing.
* **Topical Emollient:** Apply to affected skin as needed, avoiding application to large areas.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment.
## Contraindications
* Appendicitis or undiagnosed abdominal pain.
* Intestinal obstruction.
* Gastrointestinal bleeding.
* Allergy to liquid paraffin.
* Infants under 1 year of age (risk of lipid pneumonia if aspirated).
## Adverse Effects
* **Gastrointestinal:** Abdominal cramping, diarrhea, nausea, anal leakage (especially with higher doses or inadequate fluid intake).
* **Aspiration:** Risk of lipoid pneumonia, particularly in the elderly, those with impaired swallowing reflexes, or with improper administration (e.g., lying down during administration).
* **Malabsorption:** Long-term, high-dose use may interfere with the absorption of fat-soluble vitamins (A, D, E, K).
## Key Drug Interactions
* **Fat-soluble vitamins:** Reduced absorption. Separate administration by at least 2 hours.
* **Other laxatives:** Increased risk of diarrhea and electrolyte imbalance.
## Monitoring
* Monitor for bowel movements, abdominal pain, and signs of dehydration.
* Assess for anal leakage or soiling.
* In patients on long-term therapy, consider monitoring for signs of vitamin deficiency.
## Clinical Pearls
* Administer liquid paraffin at bedtime to minimize interruption of daily activities.
* Encourage adequate fluid intake (water) when using as a laxative to enhance its effectiveness and prevent dehydration.
* Avoid administration while lying down or immediately before lying down to reduce the risk of aspiration.
* If symptoms persist beyond 1 week of use, advise patient to consult a healthcare professional.
* Not intended for chronic use. If chronic constipation is present, investigate underlying causes.
