# Liquid Paraffin
## Overview
Liquid paraffin is a mineral oil used as an emollient and laxative. It acts by lubricating the stool and the intestinal wall, softening fecal matter and easing defecation.
## Primary Indications
* Constipation
## Adult Dosing
* **Oral:** 10 mL to 30 mL once daily, typically at bedtime.
* **Rectal (as enema):** May be used rectally for fecal impaction, follow specific product instructions or local protocol.
## Pediatric Dosing
* **Oral:**
* **1-5 years:** 5 mL once daily.
* **6-12 years:** 10 mL once daily.
* **Over 12 years:** Same as adult dosing.
* *Note: Use in infants and very young children should be under medical supervision due to the risk of aspiration.*
## Dose Adjustments
No dose adjustments are typically needed for renal or hepatic impairment.
## Contraindications
* Known hypersensitivity to liquid paraffin.
* Patients with appendicitis, intestinal obstruction, or fecal impaction.
* Prolonged use is generally not recommended.
* Caution in individuals with difficulty swallowing due to risk of aspiration, especially in the elderly or those with neurological impairment.
## Adverse Effects
* **Common:** Abdominal cramps, bloating, diarrhea, anal leakage.
* **Rare:** Lipid pneumonia (if aspirated), vitamin malabsorption (with chronic use).
## Key Drug Interactions
* **Mineral Oil (Oral):** May decrease the absorption of fat-soluble vitamins (A, D, E, K) and concurrently administered oral medications, particularly those with sustained-release formulations. Separate administration by at least 2 hours.
## Monitoring
* Monitor for effectiveness (bowel movements).
* Assess for signs of anal leakage, cramping, or diarrhea.
* If used chronically, consider monitoring for vitamin deficiency.
## Clinical Pearls
* Administer at bedtime to minimize anal leakage during the day.
* Avoid concurrent use with fat-soluble vitamins or oral medications that require consistent absorption.
* Educate patients about the potential for anal leakage and the importance of not lying down immediately after administration due to aspiration risk.
* Long-term, daily use is not recommended due to potential complications.
