# Liquid Paraffin
## Overview
Liquid paraffin is a mineral oil used as a laxative. It lubricates the intestinal tract, softening stool and easing passage. It is not absorbed systemically.
## Primary Indications
* Constipation
## Adult Dosing
* **Oral:** 10-30 mL once daily at bedtime. Doses can be adjusted based on response.
* Maximum dose is not strictly defined but typically kept to 30 mL daily to minimize aspiration risk.
## Pediatric Dosing
* **Oral:**
* 1-6 years: 5 mL once daily at bedtime.
* 6-12 years: 10 mL once daily at bedtime.
* Over 12 years: Use adult dosing.
* Dosing can be adjusted based on response, but caution is advised due to increased aspiration risk in children.
## Dose Adjustments
No specific dose adjustments are necessary for renal or hepatic impairment as the drug is not absorbed.
## Contraindications
* Known hypersensitivity to liquid paraffin.
* Patients with difficulty swallowing or a history of aspiration.
* Acute abdominal pain, nausea, vomiting, or other symptoms of appendicitis or undiagnosed abdominal conditions.
* Intestinal obstruction.
## Adverse Effects
* Gastrointestinal: Anal leakage, diarrhea, abdominal cramps, nausea, vomiting.
* Other: Lipid pneumonia (if aspirated), perianal irritation.
## Key Drug Interactions
* **Mineral Oil:** Can reduce the absorption of fat-soluble vitamins (A, D, E, K) and fat-soluble medications. Separate administration by at least 2 hours.
* May interfere with diagnostic imaging procedures of the abdomen (e.g., barium enema).
## Monitoring
* Monitor for bowel movement frequency and consistency.
* Assess for signs of anal leakage or perianal irritation.
* Watch for signs of aspiration, especially in pediatric patients or the elderly.
## Clinical Pearls
* Administer at bedtime to reduce the risk of anal leakage during the day.
* Avoid concurrent use with other oils or fatty foods.
* Long-term, frequent use may lead to dependence and reduced bowel motility.
* Aspiration is a serious risk and can lead to lipoid pneumonia. Patients with dysphagia or compromised gag reflexes should not use this product.
