# Liquid Paraffin
## Overview
Liquid paraffin is a mineral oil used as a laxative. It lubricates the intestinal tract, softening stool and easing its passage.
## Primary Indications
Constipation.
## Adult Dosing
* **Oral:** 10-30 mL once daily, typically at bedtime. Some sources suggest up to 45 mL daily. The exact dose may vary based on patient response and local protocol.
## Pediatric Dosing
* **Oral:** Dosing is variable and generally not recommended for children under 12 years without medical supervision. For children 6-12 years, 5-15 mL once daily may be used. For younger children, consult specific pediatric guidelines or a pediatrician.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment.
## Contraindications
* Hypersensitivity to liquid paraffin.
* Intestinal obstruction, appendicitis, or acute inflammatory bowel disease.
* Nausea, vomiting, or abdominal pain of unknown origin.
* Children under 6 months of age.
## Adverse Effects
* Most common: Anal leakage (especially with higher doses), abdominal cramping, nausea.
* Less common: Lipoid pneumonia (with chronic aspiration, particularly in the elderly or those with swallowing difficulties).
## Key Drug Interactions
* **Mineral oil can decrease the absorption of fat-soluble vitamins (A, D, E, K)** and other fat-soluble drugs. Administer at least 2 hours apart from these medications.
* May affect the absorption of other oral medications due to its lubricating effect.
## Monitoring
* Monitor for bowel movements and signs of impaction.
* Assess for anal leakage and skin irritation.
* Observe for signs of aspiration, especially in at-risk populations.
## Clinical Pearls
* Administer at bedtime to minimize daytime anal leakage.
* Advise patients to report any difficulty swallowing, coughing during administration, or severe abdominal pain.
* Long-term use is generally discouraged due to potential for vitamin malabsorption and dependence.
* Should not be taken immediately before lying down due to the risk of aspiration.
