# Liquid Paraffin
## Overview
Liquid paraffin is a mineral oil that acts as an emollient and a laxative. It lubricates the intestinal tract, softening stool and easing defecation.
## Primary Indications
* Constipation
## Adult Dosing
* **Oral:** 10-30 mL once daily or divided into 2-3 doses.
* **Rectal (enema):** As per product labeling or local protocol, typically for rectal impaction.
## Pediatric Dosing
* **Oral:** Dosing varies by age and weight; consult specific product labeling or local pediatric guidelines. As a general guideline, doses may range from 5 mL to 15 mL once daily.
* **Rectal (enema):** Consult specific product labeling or local pediatric guidelines.
## Dose Adjustments
* No specific dose adjustments are typically required for hepatic or renal impairment.
## Contraindications
* Appendicitis or undiagnosed abdominal pain
* Intestinal obstruction
* Allergy to liquid paraffin
## Adverse Effects
* **Common:** Anal leakage, diarrhea, abdominal cramping.
* **Less Common:** Nausea, vomiting, skin rashes.
* **Rare:** Vitamin malabsorption (especially fat-soluble vitamins A, D, E, K) with chronic, frequent use. Lipid pneumonia if aspirated.
## Key Drug Interactions
* **Fat-soluble vitamins (A, D, E, K):** Liquid paraffin can decrease their absorption. Consider separate administration or supplementation if chronic use is anticipated.
* **Other oral medications:** May affect absorption due to its physical presence in the GI tract. Administer at least 2 hours apart from other oral medications.
## Monitoring
* Bowel movement frequency and consistency.
* Signs of vitamin deficiency with chronic use.
* Signs of aspiration (e.g., cough, fever, shortness of breath) if oral intake is challenging or in patients with dysphagia.
## Clinical Pearls
* Administer undiluted or mixed with water or juice.
* Advise patients to avoid lying down immediately after taking liquid paraffin orally to minimize the risk of aspiration.
* Use with caution in the elderly, those with swallowing difficulties, or those who are debilitated due to the risk of aspiration and lipid pneumonia.
* Limit duration of use to avoid potential vitamin malabsorption and dependency.
