# Liquid Paraffin
## Overview
Liquid paraffin is a mineral oil used as an emollient and laxative. It lubricates the intestinal tract, softening stool and facilitating bowel movements.
## Primary Indications
* Constipation
## Adult Dosing
* **Laxative:** 10-30 mL once daily, typically at bedtime.
* **Emollient:** Apply topically to affected skin areas as needed.
## Pediatric Dosing
Dosing for children should be determined by a healthcare provider based on age and weight. For constipation, typical doses are:
* **1-5 years:** 5 mL once daily.
* **6-12 years:** 10 mL once daily.
* **Over 12 years:** 15-30 mL once daily.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment.
## Contraindications
* Hypersensitivity to liquid paraffin.
* Appendicitis or symptoms of acute abdominal conditions.
* Intestinal obstruction.
* Uncontrolled diabetes mellitus (due to potential for aspiration, especially in infants and elderly).
* Difficulty swallowing (risk of aspiration).
## Adverse Effects
* **Gastrointestinal:** Abdominal cramping, bloating, diarrhea, nausea, anal leakage.
* **Systemic (with chronic use or high doses):** Fat-soluble vitamin malabsorption (A, D, E, K), lipoid pneumonia (if aspirated), perianal irritation.
## Key Drug Interactions
* **Fat-soluble vitamins (A, D, E, K):** Long-term use may decrease absorption.
* **Other medications:** May delay the absorption of other orally administered drugs.
## Monitoring
* Bowel movement frequency and consistency.
* Signs of vitamin deficiencies with chronic use.
* Respiratory status if aspiration is suspected.
## Clinical Pearls
* Administer at bedtime to minimize interference with appetite and nutrient absorption.
* Advise patients to take upright and avoid lying down immediately after administration to reduce aspiration risk.
* Long-term, daily use is generally not recommended due to potential for dependency and vitamin malabsorption.
