# Liquid Paraffin
## Overview
Liquid paraffin is a mineral oil used as a lubricant laxative. It softens fecal mass and eases defecation.
## Primary Indications
Constipation.
## Adult Dosing
* **Oral:** 10 mL to 20 mL once daily, taken at bedtime.
* **Maximum:** Typically not explicitly defined but should be used judiciously to avoid dependence or side effects.
## Pediatric Dosing
* **Children 6-12 years:** 5 mL once daily at bedtime.
* **Children 1-5 years:** 2.5 mL to 5 mL once daily at bedtime.
* **Under 1 year:** Use is generally not recommended due to risk of aspiration and potential for systemic absorption. If used, consult a pediatrician; typical doses may be around 1-2.5 mL once daily.
## Dose Adjustments
No specific dose adjustments are typically required based on renal or hepatic impairment. However, prolonged use should be avoided in all patient populations.
## Contraindications
* Hypersensitivity to liquid paraffin.
* Abdominal pain of unknown cause.
* Nausea or vomiting.
* Intestinal obstruction or suspected obstruction.
* Difficulty swallowing (risk of aspiration).
* Concurrent use of docusate.
## Adverse Effects
* Anal leakage (most common, especially with higher doses).
* Abdominal cramping, bloating, diarrhea.
* Nausea, vomiting.
* Lipoid pneumonia (rare, associated with chronic aspiration).
* Vitamin malabsorption (fat-soluble vitamins A, D, E, K) with chronic use.
## Key Drug Interactions
* **Fat-soluble vitamins (A, D, E, K):** Decreased absorption with chronic use.
* **Docusate:** Concurrent use is contraindicated due to increased risk of systemic absorption and toxicity.
* **Other laxatives:** Avoid concurrent use of stimulant laxatives to prevent electrolyte imbalances.
## Monitoring
* Bowel movement frequency and consistency.
* Signs of abdominal pain, distension, or obstruction.
* Evaluate for long-term use necessity; aim for short-term management.
## Clinical Pearls
* Administer at bedtime to minimize anal leakage during waking hours.
* Advise patients to drink a full glass of water with each dose.
* Avoid administration in a supine position due to aspiration risk.
* Long-term use can lead to dependence and impaired absorption of fat-soluble vitamins.
* Consider alternative treatments for chronic constipation.
