# Isoxsuprine Hydrochloride
## Overview
Isoxsuprine hydrochloride is a vasodilator that may improve peripheral circulation. Its efficacy in various vascular conditions is not well-established by robust clinical trials.
## Primary Indications
* Peripheral vascular disease (e.g., intermittent claudication, Raynaud's disease).
* Cerebral vascular insufficiency.
## Adult Dosing
* **Oral:** The typical dose is 10-20 mg three to four times daily. Doses up to 40 mg four times daily have been reported, but higher doses are generally not recommended due to lack of proven benefit and increased risk of adverse effects.
## Pediatric Dosing
* There is no established pediatric dosing for isoxsuprine hydrochloride.
## Dose Adjustments
* No specific dose adjustments are routinely recommended for hepatic or renal impairment. However, caution is advised in patients with significant organ dysfunction.
## Contraindications
* Hypersensitivity to isoxsuprine hydrochloride.
* Do not use in patients who have had a recent myocardial infarction.
## Adverse Effects
* **Common:** Nausea, vomiting, gastrointestinal distress, dizziness, weakness, nervousness, rash, flushing.
* **Less Common:** Hypotension, tachycardia, drowsiness, blurred vision.
## Key Drug Interactions
* **Antihypertensives:** Isoxsuprine may potentiate the effects of antihypertensive medications, leading to hypotension.
* **Epinephrine:** May antagonize the vasodilator effects of isoxsuprine.
## Monitoring
* Blood pressure: Monitor for hypotension, especially at the start of therapy or with dose increases.
* Symptoms: Assess for improvement in peripheral circulation symptoms.
* Adverse effects: Monitor for any new or worsening side effects.
## Clinical Pearls
* Isoxsuprine hydrochloride should be administered with meals to minimize gastrointestinal upset.
* Due to limited evidence supporting its efficacy and potential for adverse effects, its use should be carefully considered against alternative treatments.
* Inform patients that therapeutic effects may take several weeks to become apparent.
