# Isoxsuprine Hydrochloride
## Overview
Isoxsuprine hydrochloride is a peripheral vasodilator. Its mechanism of action involves direct relaxation of vascular smooth muscle, leading to vasodilation.
## Primary Indications
* Intermittent claudication secondary to peripheral vascular disease.
* Raynaud's disease.
## Adult Dosing
* **Initial Dose:** 10-20 mg orally every 6-8 hours.
* **Maintenance Dose:** 20 mg orally every 6-8 hours. Doses may be increased cautiously based on patient response and tolerance.
* **Maximum Dose:** Typically not to exceed 40 mg every 6-8 hours.
## Pediatric Dosing
* Limited data available for pediatric use. Dosing should be determined by a qualified clinician, often starting at lower doses and titrating carefully.
## Dose Adjustments
* No specific dose adjustments are routinely recommended for renal or hepatic impairment, but caution and close monitoring are advised due to potential accumulation.
## Contraindications
* Do not use in patients with recent arterial bleeding or in those who have experienced a recent coronary occlusion.
* Hypersensitivity to isoxsuprine hydrochloride.
## Adverse Effects
* **Common:** Nausea, vomiting, gastrointestinal distress, dizziness, weakness, nervousness, flushing, and a rapid heartbeat (tachycardia).
* **Less Common:** Hypotension, skin rash, and abdominal pain.
## Key Drug Interactions
* **Antihypertensives:** Isoxsuprine can potentiate the hypotensive effects of antihypertensive medications.
* **Vasodilators:** Concomitant use with other vasodilators may increase the risk of hypotension.
## Monitoring
* Blood pressure and heart rate.
* Symptoms of peripheral vascular disease (e.g., walking distance, pain).
* Signs of adverse effects.
## Clinical Pearls
* The efficacy of isoxsuprine hydrochloride for intermittent claudication is controversial and not definitively established by robust clinical trials.
* It is often used as an adjunct to other therapies, including exercise and lifestyle modifications.
* Dosing should be individualized based on patient response and tolerance.
