# Isoxsuprine hydrochloride
## Overview
Isoxsuprine hydrochloride is a peripheral vasodilator.
## Primary Indications
* Peripheral vascular disease (e.g., intermittent claudication, Buerger's disease) to improve blood flow.
* Cerebrovascular insufficiency.
* Historically used in premature labor, but this is generally not recommended due to lack of efficacy and safety concerns.
## Adult Dosing
* **Intermittent claudication and peripheral vascular disease:** 10-20 mg orally every 3-4 times daily.
* **Cerebrovascular insufficiency:** 10-20 mg orally every 3-4 times daily.
Dosage adjustments may be based on patient response and tolerance, not to exceed 40 mg daily.
## Pediatric Dosing
There is no established pediatric dosing for isoxsuprine hydrochloride.
## Dose Adjustments
No specific dose adjustments are routinely recommended for renal or hepatic impairment. However, caution is advised, and dose reduction may be considered based on clinical assessment.
## Contraindications
* Hypersensitivity to isoxsuprine hydrochloride.
* Use in patients with congenital heart defects or arrhythmias.
## Adverse Effects
* **Cardiovascular:** Hypotension, tachycardia, dizziness, flushing, palpitations.
* **Gastrointestinal:** Nausea, vomiting, abdominal distress.
* **Other:** Weakness, nervousness, rash.
## Key Drug Interactions
* **Antihypertensives:** Isoxsuprine may potentiate the hypotensive effects of antihypertensive medications.
* **Sympathomimetic agents:** Concomitant use may lead to additive cardiovascular effects (e.g., increased heart rate, blood pressure).
## Monitoring
* Blood pressure and heart rate.
* Signs and symptoms of peripheral vascular disease improvement.
* Signs of adverse effects.
## Clinical Pearls
* Effectiveness in treating peripheral vascular disease is debated, and evidence is limited.
* Should be used with caution in patients with hypotension or cardiac conditions.
* Administration with food may reduce gastrointestinal upset.
