# Isoxsuprine Hydrochloride
## Overview
Isoxsuprine hydrochloride is a vasodilator with beta-adrenergic agonist activity. It is thought to relax smooth muscle in blood vessels, leading to vasodilation and improved peripheral circulation.
## Primary Indications
* Treatment of peripheral vascular disease, such as intermittent claudication and Raynaud's disease.
* Historically used to prevent premature labor (tocolytic), but this use is generally not recommended due to risks and availability of more effective agents.
## Adult Dosing
* **Peripheral Vascular Disease:** 10 mg to 20 mg orally every 3 to 4 times daily.
* **Maximum Dose:** Doses up to 40 mg orally every 3 to 4 times daily have been used, but higher doses may increase the risk of adverse effects.
## Pediatric Dosing
There is no established pediatric dosing for isoxsuprine hydrochloride. Its use in children is generally not recommended.
## Dose Adjustments
* **Hepatic Impairment:** No specific guidelines are available, but caution is advised as the drug is metabolized in the liver.
* **Renal Impairment:** No specific guidelines are available, but caution is advised as the drug and its metabolites are renally excreted.
## Contraindications
* Hypersensitivity to isoxsuprine hydrochloride.
* Use in patients with a history of glaucoma.
* Use in patients with recent myocardial infarction.
* Use in patients with cardiac arrhythmias or tachyarrhythmias.
## Adverse Effects
* **Cardiovascular:** Hypotension, tachycardia, flushing, dizziness, lightheadedness.
* **Gastrointestinal:** Nausea, vomiting, abdominal distress, constipation.
* **Neurological:** Weakness, nervousness.
* **Other:** Rash, blurred vision.
## Key Drug Interactions
* **Antihypertensives:** Isoxsuprine may potentiate the hypotensive effects of antihypertensive medications.
* **Sympathomimetic Agents:** Concomitant use may increase the risk of cardiovascular side effects.
## Monitoring
* Blood pressure and heart rate.
* Signs and symptoms of peripheral vascular disease.
* Patient-reported adverse effects.
## Clinical Pearls
* The efficacy of isoxsuprine for peripheral vascular disease is not definitively established, and its use is limited.
* Patients should be advised to rise slowly from a sitting or lying position to minimize orthostatic hypotension.
* Discontinue if paradoxical vasospasm occurs.
