# Isoxsuprine Hydrochloride
## Overview
Isoxsuprine hydrochloride is a vasodilator and smooth muscle relaxant. Its exact mechanism of action is not fully understood, but it is believed to cause relaxation of vascular smooth muscle, leading to vasodilation.
## Primary Indications
* Intermittent claudication secondary to peripheral vascular disease.
* Vasospasm associated with peripheral vascular disease.
## Adult Dosing
* **Intermittent Claudication/Peripheral Vascular Disease:** 10 mg to 20 mg orally three to four times daily.
* **Maximum Dose:** Not explicitly defined, but doses above 20 mg four times daily are generally not recommended.
## Pediatric Dosing
* There is no established pediatric dosing for isoxsuprine hydrochloride.
## Dose Adjustments
* No specific dose adjustments are typically required for hepatic or renal impairment, however, caution should be exercised in patients with severe disease.
## Contraindications
* Hypersensitivity to isoxsuprine hydrochloride.
* In patients who have experienced a recent arterial embolus.
* In patients with severe cardiac disease.
## Adverse Effects
* **Common:** Nausea, vomiting, gastrointestinal distress, dizziness, weakness, flushing, rash.
* **Less Common:** Hypotension, tachycardia, drowsiness, nervousness, abdominal distress, constipation.
## Key Drug Interactions
* **Antihypertensives:** Isoxsuprine may potentiate the effects of antihypertensive medications, leading to additive hypotension. Monitor blood pressure closely.
* **Other Vasodilators:** Concurrent use with other vasodilators may increase the risk of hypotension.
## Monitoring
* Monitor for signs and symptoms of peripheral vascular disease improvement.
* Monitor for adverse effects, particularly hypotension, dizziness, and gastrointestinal upset.
* Monitor blood pressure.
## Clinical Pearls
* Isoxsuprine is generally not considered a first-line agent for peripheral vascular disease due to limited evidence of efficacy and potential for side effects.
* Optimal therapeutic effect may not be observed for several weeks.
* Discontinue if gastric irritation or other adverse effects occur.
