# Isoxsuprine Hydrochloride
## Overview
Isoxsuprine hydrochloride is a peripheral vasodilator.
## Primary Indications
* Adjunctive therapy in peripheral arterial disease (PAD) for intermittent claudication.
* Relief of peripheral vascular spasms.
**Note:** The efficacy of isoxsuprine for PAD is controversial and not widely supported by current evidence.
## Adult Dosing
* **Intermittent Claudication/Peripheral Vascular Spasms:** 10-20 mg orally 3-4 times daily.
* **Maximum Dose:** Not well-established, but doses exceeding 20 mg per administration or 80 mg per day are generally not recommended.
## Pediatric Dosing
* No established pediatric dosing. Use in pediatric patients is not recommended.
## Dose Adjustments
* No specific dose adjustments are typically recommended for hepatic or renal impairment. However, caution should be exercised due to potential for reduced metabolism or excretion.
## Contraindications
* Hypersensitivity to isoxsuprine.
* Use in patients with congenital heart disease.
* Use in patients where a vasodilation effect would be dangerous (e.g., following recent hemorrhage, glaucoma).
## Adverse Effects
* **Common:** Nausea, vomiting, gastrointestinal upset, dizziness, weakness, flushing, headache, nervousness, rash.
* **Less Common:** Hypotension, tachycardia, palpitations, drowsiness, abdominal distress.
## Key Drug Interactions
* **Antihypertensives:** Isoxsuprine may potentiate the hypotensive effects of antihypertensive medications.
* **Epinephrine:** Isoxsuprine may block the vasoconstrictor effects of epinephrine.
## Monitoring
* Monitor blood pressure and heart rate for signs of hypotension or tachycardia.
* Assess for improvement in claudication symptoms and peripheral circulation.
* Monitor for adverse effects.
## Clinical Pearls
* Isoxsuprine's clinical benefit in PAD is questionable. Consider alternative therapies with better evidence.
* Administer with food or milk to minimize gastrointestinal upset.
* Avoid use in situations where vasodilation is contraindicated.
