# Isoxsuprine hydrochloride
## Overview
Isoxsuprine hydrochloride is a peripheral vasodilator. Its exact mechanism of action is not fully understood, but it is thought to relax vascular smooth muscle.
## Primary Indications
* Treatment of peripheral vascular disease.
* Intermittent claudication.
## Adult Dosing
* **Oral:** 10 mg to 20 mg three to four times daily.
* **Maximum:** Doses up to 40 mg four times daily have been used in severe cases, but efficacy and safety at higher doses are not well established.
## Pediatric Dosing
* No established pediatric dosing. Use is generally not recommended.
## Dose Adjustments
* No specific dose adjustments are typically recommended for renal or hepatic impairment, but caution should be exercised due to limited data.
## Contraindications
* Hypersensitivity to isoxsuprine.
* Should not be administered intravenously or intramuscularly.
## Adverse Effects
* **Common:** Nausea, vomiting, gastric distress, dizziness, weakness, flushing, nervousness, and rash.
* **Less common:** Hypotension, blurred vision, and shortness of breath.
## Key Drug Interactions
* **Antihypertensives:** Isoxsuprine may potentiate the effects of antihypertensive medications, leading to excessive hypotension. Monitor blood pressure closely.
* **Other vasodilators:** Concurrent use may increase the risk of hypotension.
## Monitoring
* Monitor blood pressure and heart rate.
* Assess for improvement in symptoms of peripheral vascular disease.
* Monitor for adverse effects.
## Clinical Pearls
* The efficacy of isoxsuprine in treating peripheral vascular disease is controversial and not consistently supported by robust clinical evidence.
* Use should be considered as adjunctive therapy and not a substitute for other established treatments like smoking cessation, exercise, and wound care.
* Paradoxical bronchospasm has been reported in patients with asthma or emphysema.
