# Hyaluronic acid
## Overview
Hyaluronic acid (HA) is a naturally occurring glycosaminoglycan found in various connective tissues, synovial fluid, and ocular tissues. It acts as a lubricant and shock absorber. As a pharmaceutical agent, it is available in various formulations for different routes of administration.
## Primary Indications
* **Intra-articular injection:** Osteoarthritis of the knee.
* **Ophthalmic solution:** Ocular surface disease (e.g., dry eye), intraocular lens insertion during cataract surgery.
* **Topical:** Wound healing, osteoarthritis (oral or topical formulations exist but have less robust evidence).
## Adult Dosing
* **Osteoarthritis of the knee (intra-articular):**
* **Common formulations:** Typically 20 mg/2 mL (10 mg/mL) or 60 mg/6 mL (10 mg/mL) injected intra-articularly once a week for 3-5 weeks. Some preparations allow a single 60 mg injection.
* **Maximum:** Treatment courses may be repeated every 6 months.
* **Ophthalmic (dry eye):**
* **Common formulations:** 0.1% to 0.2% solutions.
* **Dosing:** 1-2 drops instilled into the affected eye(s) as needed, up to 4-6 times daily. Some higher concentrations may be used less frequently.
* **Intraocular surgery (viscosurgical device):**
* **Dosing:** Administered intraocularly by a surgeon during procedures like cataract surgery to maintain space and protect tissues. Specific volume and concentration depend on the surgical technique and product.
## Pediatric Dosing
* **Intra-articular:** Limited data; generally not recommended or used off-label in pediatric populations for osteoarthritis.
* **Ophthalmic:** Used in pediatric patients for dry eye, similar to adults (e.g., 0.1% to 0.2% solution, 1-2 drops as needed). Safety and efficacy in children under a certain age may not be established by all manufacturers.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment, as HA has minimal systemic absorption.
## Contraindications
* Known hypersensitivity to hyaluronic acid or any component of the formulation.
* Active infection in or around the joint to be injected (for intra-articular preparations).
* Skin infection over the injection site (for intra-articular preparations).
## Adverse Effects
* **Intra-articular:** Localized pain, swelling, stiffness, warmth, redness, itching, or transient effusion at the injection site. Rare: allergic reactions, joint infection.

* **Systemic:** Very rare.
