# Hyaluronic Acid
## Overview
Hyaluronic acid (HA) is a naturally occurring glycosaminoglycan found in various connective tissues, synovial fluid, and ocular tissues. It acts as a lubricant and shock absorber. Pharmaceutical preparations are typically derived from microbial fermentation.
## Primary Indications
* **Ophthalmology:** Intraocular lens insertion, visco-surgery (cataract extraction, corneal transplantation), dry eye disease.
* **Orthopedics:** Osteoarthritis of the knee (intra-articular injection).
* **Dermatology:** Cosmetic filler for facial wrinkles.
* **Wound Healing:** Topical formulations for wound management.
## Adult Dosing
* **Ophthalmology (Viscosurgery):** Typically 0.5 mL to 2 mL injected into the anterior chamber during surgery. Specific volume depends on the surgical procedure and product concentration.
* **Ophthalmology (Dry Eye):** Ophthalmic solutions/gels: 1 drop instilled into the affected eye(s) as needed, or as directed by prescribing information (concentrations vary, e.g., 0.1%, 0.2%, 0.4%, 1%). Frequency can range from once daily to multiple times per day.
* **Orthopedics (Osteoarthritis of the knee):** Intra-articular injection: Typically 2 mL to 3 mL (concentrations vary, e.g., 10 mg/mL, 20 mg/mL) into the knee joint. May be given as a single injection or a series of 3-5 weekly injections. Specific protocol depends on the product and local guidelines.
* **Dermatology (Cosmetic Filler):** Subcutaneous injection into facial wrinkles. Volume and technique are product-specific and administered by trained professionals.
## Pediatric Dosing
Dosing in pediatric populations is not well-established for most indications and generally not recommended without specific clinical justification and expert consultation. Use in ophthalmic preparations for dry eye may be considered on a case-by-case basis with appropriate monitoring.
## Dose Adjustments
* **Renal/Hepatic Impairment:** No specific dose adjustments are typically required for systemically absorbed HA, as it is metabolized similarly to endogenous HA. However, local administration requires consideration of potential effects at the site of injection or application.
## Contraindications

* **Orthopedics:** Concurrent use of local anesthetics may increase the risk of systemic absorption and toxicity, though this is less common with HA. Avoid concurrent intra-articular administration of other viscoelastic agents.
## Monitoring
* **Ophthalmology:** Intraocular pressure, visual acuity, signs of infection or inflammation.
* **Orthopedics:** Signs of local infection, joint effusion, pain, and functional improvement.
* **Dermatology:** Injection site for signs of infection, inflammation, or granuloma formation.
## Clinical Pearls
* HA is a viscoelastic substance; therefore, careful technique is required during administration to avoid complications.
* For ophthalmic use, different concentrations and molecular weights exist, influencing viscosity and duration of action.
* Intra-articular HA for osteoarthritis is typically considered after failure of conservative measures like NSAIDs or physical therapy. It is not a substitute for weight management or exercise.
* Effectiveness and duration of cosmetic filler effects vary significantly between individuals.
