# Hyaluronic Acid
## Overview
Hyaluronic acid (HA) is a naturally occurring glycosaminoglycan found in various connective tissues, including synovial fluid, skin, and eyes. It acts as a lubricant and shock absorber. As a pharmaceutical agent, it is used to supplement or replace endogenous HA in specific applications.
## Primary Indications
* **Intra-articular injection:** Osteoarthritis (knee, hip).
* **Ophthalmic surgery:** Viscosurgical device for cataract surgery, intraocular lens implantation, and corneal transplantation.
* **Wound healing:** Topical formulations for burns, ulcers.
* **Dermal fillers:** Aesthetic correction of wrinkles and facial contouring (often cross-linked formulations, which are distinct from injectable viscosupplementation).
## Adult Dosing
* **Intra-articular (Osteoarthritis):**
* **Knee:** Typically 2 mL (20 mg) of 1% solution administered intra-articularly once weekly for 3 weeks. Some products may allow a single injection or up to 5 injections. Maximum total volume per injection is usually 2 mL. A course of treatment can be repeated every 6 months.
* **Hip:** Dosing varies by product; consult specific product labeling. Often involves a single injection of a higher concentration/volume.
* **Ophthalmic (Viscosurgical Device):**
* Administered intraocularly by a surgeon during ophthalmic procedures. Volume and concentration depend on the specific product and surgical needs, typically ranging from 0.5 mL to 2 mL.
* **Topical (Wound Healing):**
* Applied directly to wound or burn as a gel, spray, or dressing. Frequency and duration depend on the wound characteristics and product formulation.
## Pediatric Dosing
* **Intra-articular:** Use in pediatric patients for osteoarthritis is generally not established and should be guided by expert opinion and available literature, if any.
* **Ophthalmic:** Safety and efficacy in pediatric patients for ophthalmic surgical use are not well-established.
## Dose Adjustments
* **Renal Impairment:** No specific dose adjustments are typically required for intra-articular or ophthalmic use as systemic absorption is minimal.
* **Hepatic Impairment:** No specific dose adjustments are typically required.
## Contraindications
* Known hypersensitivity to hyaluronic acid or any component of the formulation.
* Active infection at or near the injection site.
* Skin infection over the proposed injection site.
* Severe inflammation or effusion in the target joint (may necessitate aspiration first).
## Adverse Effects
* **Intra-articular:** Localized pain, swelling, stiffness, joint effusion, transient inflammatory reactions, arthralgia, back pain, headache. Rarely, anaphylactic reactions.
* **Ophthalmic:** Transient intraocular pressure increase, anterior chamber inflammation, corneal edema, and transient blurred vision.
* **Topical:** Local irritation, redness, or allergic reactions.
## Key Drug Interactions
* **Intra-articular:** Concurrent use of other intra-articular medications, particularly corticosteroids, should be carefully considered due to potential effects on joint health and HA efficacy. Data on specific interactions are limited.
* **Ophthalmic:** Concurrent use with other intraocular medications may alter their absorption or distribution.
## Monitoring
* **Intra-articular:** Monitor for signs and symptoms of infection or allergic reaction. Assess pain and functional improvement.
* **Ophthalmic:** Monitor intraocular pressure and signs of inflammation post-operatively.
## Clinical Pearls
* For intra-articular injections, aspiration of synovial fluid may be beneficial before administration to reduce joint pressure and rule out infection.
* Warm the syringe to room temperature before injection to improve flowability.
* Avoid vigorous use of the affected joint for 24-48 hours after intra-articular injection.
* Topical HA formulations are often used as adjuncts to wound care and may help maintain a moist wound environment.
