# Hyaluronic acid
## Overview
Hyaluronic acid (HA) is a naturally occurring high-molecular-weight glycosaminoglycan found in connective tissue, skin, and synovial fluid. Clinical formulations are used for intra-articular supplementation (viscosupplementation), ophthalmic procedures, and topically for wound healing or dermatologic aesthetics.
## Primary Indications
* **Osteoarthritis (OA) of the knee:** Pain management in patients failing conservative therapy.
* **Ophthalmic surgery:** Maintaining anterior chamber depth (viscoelastic agent).
* **Dermatology:** Dermal filler for tissue augmentation; topical moisturizer for wound care.
## Adult Dosing
* **Intra-articular (Knee OA):** Dosing and frequency vary significantly by specific product (e.g., Hyalgan, Synvisc, Euflexxa).
* **Common regimen:** 2 mL (10–20 mg per mL) injected intra-articularly once weekly for 3 to 5 weeks.
* **Single-injection products (e.g., Durolane):** 60 mg (3 mL) as a single intra-articular injection.
* **Note:** Always consult the specific product package insert, as dosing is non-interchangeable between brands.
## Pediatric Dosing
Safety and efficacy are not established for intra-articular use in patients <18 years. Use is generally restricted to ophthalmic surgery per surgeon discretion under specialized protocols.
## Dose Adjustments
* **Renal/Hepatic impairment:** No specific adjustments required for intra-articular or topical administration.
* **Geriatric:** Use cautiously in patients with known inflammatory joint conditions or active infections.
## Contraindications
* **Allergy:** Known hypersensitivity to hyaluronan preparations or avian proteins (for products derived from avian sources).
* **Infection:** Active joint infection, skin disease, or infection in the area of the injection site.
* **Known systemic bleeding disorders** (for intra-articular use).
## Adverse Effects
* **Intra-articular:** Injection site pain, joint swelling/effusion, arthralgia, pruritus, and rarely, pseudoseptic arthritis (acute inflammatory reaction).
* **Ophthalmic:** Transient increased intraocular pressure.
* **Dermal:** Bruising, erythema, edema, or nodules at the injection site.
## Key Drug Interactions
There are no clinically significant systemic drug-drug interactions, as hyaluronic acid is not absorbed systemically in meaningful quantities via intra-articular or topical routes. Avoid concomitant intra-articular injection of quaternary ammonium salts (e.g., benzalkonium chloride), as they may cause HA to precipitate.
## Monitoring
* **Post-Injection:** Monitor for acute joint swelling, pain, or symptoms suggestive of septic arthritis (fever, systemic chills) within the first 48 hours.
* **Efficacy:** Assess pain levels and joint range of motion 4–8 weeks post-completion of the injection cycle.
## Clinical Pearls
* **Variable Efficacy:** Meta-analyses show variable results for knee OA; clinicians should manage patient expectations regarding the time to onset (often 4+ weeks) and duration of benefit (up to 6 months).
* **Source Sensitivity:** Ensure patients are screened for egg or avian protein allergies if using avian-derived products.
* **Injection Technique:** Intra-articular administration must be performed under strict aseptic conditions. Use of ultrasound guidance is recommended to ensure intra-articular placement.
* **Storage:** Most commercially available intra-articular syringes must be stored at controlled room temperature and protected from freezing.
