# Hyaluronic Acid
## Overview
Hyaluronic acid (HA) is a naturally occurring high-molecular-weight glycosaminoglycan found in connective tissue, synovial fluid, and the eye. It functions as a lubricant, shock absorber, and structural component of the extracellular matrix. Therapeutic formulations vary widely by site of administration (intra-articular, ophthalmic, topical, or intranasal).
## Primary Indications
* **Intra-articular:** Osteoarthritis (OA) of the knee (when non-pharmacologic therapy and simple analgesics fail).
* **Ophthalmic:** Dry eye syndrome, surgical adjunct for ocular surgery (viscoelastic agent).
* **Topical/Dermal:** Wound healing, dermal fillers for aesthetic rejuvenation, facial volume restoration.
* **Intranasal:** Moisturizing nasal mucosa.
## Adult Dosing
* **Intra-articular (Knee OA):** Dosing is dependent on the specific product (e.g., Synvisc-One, Euflexxa, Hyalgan).
* *Single-injection products (e.g., Synvisc-One):* 48 mg/6 mL injected once into the intra-articular space.
* *Multi-injection products (e.g., Hyalgan):* 20 mg/2 mL injected weekly for 3–5 weeks.
* *Note:* Maximum frequency depends on local clinical protocol and product labeling; generally, do not repeat courses more frequently than every 6 months.
* **Ophthalmic:** 0.1–0.3% solution applied to the affected eye(s) 1–4 times daily as needed.
## Pediatric Dosing
Safety and efficacy have not been established for intra-articular use in patients <18 years. Use is generally restricted to ophthalmic or topical applications for minor wound/eye irritation using concentrations labeled for pediatric OTC use.
## Dose Adjustments
* **Renal/Hepatic:** No formal dose adjustments required, as systemic absorption is negligible for most formulations.
* **Geriatric:** Use with caution in patients with severe inflammatory knee effusions; aspiration of effusion should precede injection.
## Contraindications
* Known hypersensitivity to hyaluronan preparations (avian-derived products may contain trace egg/avian proteins; avoid in severe allergy).
* Active joint infection, skin infection, or inflammation in the area of the injection site.
* Severe inflammatory arthritis (e.g., rheumatoid arthritis).
## Adverse Effects
* **Local (Injection):** Arthralgia, joint swelling, stiffness, or effusion (pseudo-septic reaction).
* **Ophthalmic:** Transient blurred vision, ocular irritation/stinging.
* **Systemic:** Rare reports of hypersensitivity reactions, rash, or pruritus.
## Key Drug Interactions
* **Disinfectants:** Do not use quaternary ammonium salts (e.g., benzalkonium chloride) for skin preparation if injecting HA, as HA can precipitate in their presence.
* **Clinical Note:** No significant systemic drug-drug interactions due to limited systemic bioavailability.
## Monitoring
* Monitor for signs of local inflammation or "flare" following intra-articular injection (typically occurs within 24–72 hours).
* Assess range of motion and pain scores at follow-up visits to determine efficacy.
## Clinical Pearls
* **Efficacy:** Evidence for symptomatic relief in knee OA is heterogeneous; outcomes are often most favorable in mild-to-moderate OA.
* **Injection Technique:** Aseptic technique is mandatory to prevent septic arthritis. If a significant joint effusion is present, aspirate the fluid prior to injection.
* **Patient Education:** Instruct patients to avoid strenuous weight-bearing activity (e.g., jogging, heavy lifting) for 48 hours post-injection.
* **Source Dependency:** Products may be avian-derived (e.g., Hyalgan) or bio-fermented (non-avian); verify patient allergies if using avian-derived products.
