# Hepamerz (l-Ornithine l-Aspartate)
## Overview
Hepamerz is a combination of two amino acids, l-ornithine and l-aspartate, which are thought to play a role in the detoxification of ammonia in the liver. It is used in the management of hyperammonemia associated with liver insufficiency.
## Primary Indications
* Treatment of latent and manifest hepatic encephalopathy (HE).
* Supportive treatment in acute and chronic liver diseases.
## Adult Dosing
* **Hepatic Encephalopathy:** Typically administered as an intravenous (IV) infusion.
* **Initial dose:** 5 grams IV over 30-60 minutes, 3 times daily.
* **Severe cases:** Doses up to 10 grams IV every 8 hours (30 grams daily) have been used, but this requires careful monitoring.
* **Duration:** Usually for 7-10 days or until symptoms improve.
* **Oral formulation:** When available and appropriate, can be used for maintenance therapy.
* **Dose:** 3 grams orally, 2-3 times daily.
*Note: Specific dosing regimens can vary based on the severity of hepatic encephalopathy and local protocols. Consult institutional guidelines.*
## Pediatric Dosing
* Pediatric dosing for Hepamerz is not well-established, and its use in children should be approached with caution. Clinical decisions should be based on individual patient factors and expert consultation.
## Dose Adjustments
* No specific dose adjustments are typically recommended for renal or hepatic impairment, as the drug is primarily metabolized by the liver and its metabolites are excreted via the kidneys. However, caution and close monitoring are advised in patients with severe renal or hepatic dysfunction.
## Contraindications
* Known hypersensitivity to l-ornithine or l-aspartate.
* Severe renal failure (creatinine clearance < 25 mL/min).
## Adverse Effects
* **Common:** Nausea, vomiting, diarrhea, abdominal pain, flatulence.
* **Less Common:** Allergic reactions (rash, itching), dizziness.
## Key Drug Interactions
* No significant drug interactions have been definitively established. However, caution should be exercised when co-administering with other medications that can affect ammonia levels or have potential hepatotoxicity.
## Monitoring
* Monitor for improvement of hepatic encephalopathy symptoms (e.g., changes in mental status, confusion, asterixis).
* Monitor electrolytes, renal function, and liver function tests.
* Monitor for adverse gastrointestinal effects.
## Clinical Pearls
* Hepamerz aims to reduce blood ammonia levels by promoting ammonia detoxification via the urea cycle (ornithine) and glutamine synthesis (aspartate).
* The oral formulation may be less effective than IV for acute management of severe hepatic encephalopathy but can be useful for maintenance.
* Response to therapy can vary; consider alternative or adjunctive treatments if clinical improvement is not observed.
