# Hepamerz (l-Ornithine l-Aspartate)
## Overview
Hepamerz is a combination of two amino acids, ornithine and aspartate, used in the management of hyperammonemia, particularly in patients with liver disease. It is believed to work by promoting the detoxification of ammonia in the liver and extrahepatic tissues.
## Primary Indications
* Treatment of latent and overt hepatic encephalopathy in patients with cirrhosis.
## Adult Dosing
* **Hepatic Encephalopathy:** The typical dose is 9 to 17.6 grams per day, administered orally in 3 divided doses. Some sources suggest intravenous administration for acute decompensation, with doses ranging from 5 grams to 20 grams every 12-24 hours, but this requires specific formulation and is not standard for the oral product.
* **Note:** Dosing is highly individualized and depends on the severity of hepatic encephalopathy and patient response.
## Pediatric Dosing
* Data for pediatric use is limited and not well-established. Use in pediatric patients should be under the guidance of a specialist and based on available literature, which is scarce.
## Dose Adjustments
* No specific dose adjustments are typically required for renal impairment, as the active components are metabolized. However, caution is advised in severe renal dysfunction due to potential accumulation of other substances.
## Contraindications
* Hypersensitivity to ornithine, aspartate, or any component of the formulation.
* Severe renal failure (serum creatinine > 3 mg/dL) is a relative contraindication due to potential accumulation.
## Adverse Effects
* Gastrointestinal disturbances: nausea, vomiting, abdominal discomfort, diarrhea, bloating.
* Rarely: headache, dizziness, fatigue, skin rash.
## Key Drug Interactions
* Limited significant drug interactions are reported. However, caution should be exercised with other drugs that affect ammonia levels or are metabolized by the liver.
## Monitoring
* Monitor for signs and symptoms of hepatic encephalopathy.
* Monitor serum ammonia levels if indicated.
* Assess renal function periodically.
* Monitor for gastrointestinal side effects.
## Clinical Pearls
* Hepamerz is often taken with meals to minimize gastrointestinal upset.
* The effectiveness is primarily seen in patients with underlying liver dysfunction.
* It is not a substitute for definitive management of liver disease.
