# Hepamerz (l-Ornithine l-Aspartate)
## Overview
Hepamerz is a combination of the amino acids l-ornithine and l-aspartate, used primarily in the management of hyperammonemia associated with liver conditions. It is believed to facilitate ammonia detoxification in the liver and peripheral tissues.
## Primary Indications
* Treatment of latent and manifest hepatic encephalopathy.
## Adult Dosing
* **Oral:** The typical dose is 3 to 6 grams (equivalent to 1 to 2 sachets or vials) three times daily. Doses can be adjusted based on individual patient response and ammonia levels.
* **Intravenous (IV):** Usually administered as an infusion. A common regimen is 5 grams (1 vial) every 8-12 hours, diluted in 500 mL of a compatible IV fluid. The total daily dose should not exceed 15 grams. Treatment duration is typically short and guided by clinical response and ammonia levels.
## Pediatric Dosing
* Dosing in pediatric patients is not well-established and should be determined by a specialist based on the child's specific condition, weight, and response. Caution is advised.
## Dose Adjustments
* No specific dose adjustments are routinely recommended for renal or hepatic impairment, as the drug is metabolized and excreted via pathways not significantly affected by these conditions. However, clinical judgment is advised.
## Contraindications
* Severe hypersensitivity to l-ornithine or l-aspartate.
* Patients with advanced stages of hepatic encephalopathy (e.g., coma stage III-IV where oral intake is not possible and IV route is not indicated without specific physician orders).
* Severe kidney disease.
## Adverse Effects
