# Hepamerz (l-ornithine l-aspartate)
## Overview
Hepamerz is a combination of two amino acids, L-ornithine and L-aspartate, used in the management of hyperammonemia. It is thought to reduce ammonia levels by stimulating ammonia detoxification in the liver and kidneys.
## Primary Indications
* Treatment of latent and manifest hepatic encephalopathy (HE), particularly in patients with cirrhosis.
## Adult Dosing
* **Hepatic Encephalopathy:** Typically administered intravenously. A common regimen is 5 grams (g) of L-ornithine and 5 g of L-aspartate infused over 30 minutes, up to three times daily.
* **Alternative Dosing:** Doses may range from 5 g to 15 g of L-ornithine and 5 g to 15 g of L-aspartate per day, depending on the severity of HE and patient response. Some protocols may use higher doses, but maximum daily doses are generally not explicitly defined, and it is essential to follow local guidelines.
## Pediatric Dosing
* There is limited published pediatric data. Dosing in pediatric patients should be individualized and determined by a specialist, often at lower doses than adults and with careful monitoring.
## Dose Adjustments
* No specific dose adjustments are routinely recommended for renal or hepatic impairment, as the drug is primarily used in patients with liver disease and is metabolized by the liver and kidneys. However, caution and close monitoring are advised in patients with severe renal dysfunction.
## Contraindications
* Hypersensitivity to L-ornithine or L-aspartate.
* Severe renal insufficiency (creatinine clearance <25 mL/min).
* Electrolyte disturbances, particularly hyperkalemia, due to the potential for potassium-containing formulations.
## Adverse Effects
* Nausea and vomiting are the most common adverse effects.
* Other potential adverse effects include abdominal pain, diarrhea, flatulence, and gastrointestinal discomfort.
* Rarely, transient increases in liver enzymes have been reported.
## Key Drug Interactions
* No significant drug interactions are well-established. However, it is prudent to monitor patients for any additive effects with other medications affecting ammonia metabolism or kidney function.
## Monitoring
* Monitor ammonia levels.
* Assess for signs and symptoms of hepatic encephalopathy.
* Monitor electrolytes, especially potassium, particularly if using parenteral formulations containing potassium.
* Assess renal function.
## Clinical Pearls
* Hepamerz is most effective when ammonia levels are elevated.
* It is often used as an adjunct to other therapies for HE, such as lactulose.
* Oral formulations are available for maintenance therapy in patients with HE.
* The duration of treatment depends on the clinical response and the underlying cause of hyperammonemia.
