# Hepamerz (l-ornithine l-aspartate)
## Overview
Hepamerz is a combination of two amino acids, L-ornithine and L-aspartate. It is believed to work by promoting the detoxification of ammonia in the liver and brain.
## Primary Indications
* Treatment of latent and overt hepatic encephalopathy (HE) in patients with cirrhosis.
## Adult Dosing
* **Intravenous (IV):**
* Typically administered as a continuous infusion of 25 grams (25,000 mg) of L-ornithine L-aspartate per day (equivalent to 2 ampoules of 500 mL of 5% solution diluted in 500 mL of IV fluid) over 24 hours.
* The daily dose can be divided into 2-3 infusions over a 24-hour period.
* The duration of treatment depends on the severity of HE and the patient's response.
* **Oral:**
* Typically 3 sachets (each containing 3g L-ornithine L-aspartate) three times daily, totaling 27 grams per day.
* Dose can be adjusted based on clinical response.
## Pediatric Dosing
* There is limited pediatric data. Dosing should be individualized and determined by a healthcare professional.
## Dose Adjustments
* No specific dose adjustments are typically required for renal or hepatic impairment, as the drug is metabolized and excreted. However, careful monitoring is still advised in patients with severe liver disease.
## Contraindications
* Hypersensitivity to L-ornithine L-aspartate or any of its excipients.
* Severe renal failure (creatinine clearance < 25 mL/min) is a relative contraindication due to potential accumulation.
## Adverse Effects
* **Common:** Nausea, vomiting, abdominal pain, diarrhea, flatulence.
* **Less Common:** Dizziness, headache, skin rash, itching.
* **Rare:** Allergic reactions.
## Key Drug Interactions
* No significant drug interactions have been widely reported. However, it is always prudent to review all concomitant medications.
## Monitoring
* Monitor for improvement in symptoms of hepatic encephalopathy (e.g., confusion, asterixis).
* Monitor electrolytes and renal function, especially in patients with pre-existing renal issues.
* Monitor for adverse effects, particularly gastrointestinal upset.
## Clinical Pearls
* Hepamerz is most effective when used in conjunction with appropriate management of the underlying liver disease and precipitating factors of HE.
* For IV administration, it is typically diluted in isotonic solutions like 5% dextrose or saline.
* Oral administration may be preferred for long-term management or when IV access is not feasible.
