# HepaMerz (L-Ornithine L-Aspartate)
## Overview
HepaMerz is a combination of two amino acids, L-ornithine and L-aspartate, used in the management of hyperammonemia associated with liver disease.
## Primary Indications
* Treatment of latent and manifest hepatic encephalopathy (HE).
## Adult Dosing
* **For latent HE:** 3 grams orally three times daily.
* **For manifest HE:** 10-20 grams intravenously (IV) daily, divided into 2-3 doses, depending on severity. The maximum daily IV dose is typically 20 grams. Oral formulations are generally not recommended for acute, severe HE.
## Pediatric Dosing
* Dosing in pediatric patients is not well-established and should be determined by a specialist.
## Dose Adjustments
* No dose adjustments are typically required for renal or hepatic impairment, as the drug is metabolized by the liver and excreted by the kidneys. However, caution is advised in severe renal impairment due to potential accumulation of ornithine and aspartate.
## Contraindications
* Hypersensitivity to L-ornithine L-aspartate or any of its excipients.
* Severe renal insufficiency (serum creatinine > 3 mg/dL).
* Rare hereditary metabolic diseases such as fructose intolerance, glucose-galactose malabsorption, or sucrase-isomaltase deficiency (for oral formulations containing sorbitol).
## Adverse Effects
* **Common:** Nausea, vomiting, abdominal discomfort, diarrhea.
* **Less Common:** Dizziness, headache, fatigue.
* **Rare:** Skin rash, itching.
## Key Drug Interactions
* No significant drug interactions have been reported.
## Monitoring
* Monitor for signs and symptoms of hepatic encephalopathy.
* Monitor electrolytes and renal function, especially in patients with pre-existing renal impairment or those receiving high doses.
## Clinical Pearls
* L-ornithine and L-aspartate are involved in the urea cycle and ammonia detoxification.
* The IV route is generally preferred for acute or severe hepatic encephalopathy.
* Oral administration can be used for maintenance therapy or in patients with latent HE.
