# Hepamerz (ornithine aspartate)
## Overview
Hepamerz is a combination of the amino acids L-ornithine and L-aspartate. It is believed to work by stimulating the synthesis of urea and glutamine in the liver, thereby promoting the detoxification of ammonia.
## Primary Indications
* Treatment of hyperammonemia associated with liver disease, including hepatic encephalopathy.
## Adult Dosing
* **Intravenous:** Typically administered as an infusion. A common regimen is 10 grams of L-ornithine L-aspartate (equivalent to 5 grams L-ornithine and 5 grams L-aspartate) infused over 30-60 minutes, once to three times daily. The total daily dose should not exceed 30 grams. The duration of treatment depends on the severity of the condition and patient response.
* **Oral:** Typically 3-6 grams of L-ornithine L-aspartate administered orally up to three times daily. The total daily dose should not exceed 18 grams.
## Pediatric Dosing
* Dosing in pediatric patients is not well-established and should be individualized based on clinical assessment and response. Caution is advised.
## Dose Adjustments
* No specific dose adjustments are typically required for renal or hepatic impairment, as the drug is metabolized and excreted through the liver. However, close monitoring is warranted in patients with severe liver dysfunction.
## Contraindications
* Hypersensitivity to ornithine, aspartate, or any component of the formulation.
* Severe renal impairment (creatinine clearance < 25 mL/min), although this is debated and requires careful consideration.
* Lactic acidosis.
## Adverse Effects
