# Hepamerz (l-Ornithine l-Aspartate)
## Overview
Hepamerz is a combination of the amino acids l-ornithine and l-aspartate. It is thought to reduce ammonia levels by promoting its conversion into urea (ornithine cycle) and by facilitating the detoxification of ammonia in peripheral tissues.
## Primary Indications
* Treatment of latent and manifest hepatic encephalopathy (HE).
## Adult Dosing
* **Acute HE:** 5 g (1 sachet or 1 vial) three times daily, diluted in a sufficient amount of fluid and administered orally or intravenously.
* **Chronic HE:** 5 g (1 sachet or 1 vial) twice to three times daily, diluted in a sufficient amount of fluid and administered orally or intravenously.
* **Maximum Dose:** Generally, up to 30 g per day has been used in some clinical trials for severe HE, but standard recommended doses are lower.
## Pediatric Dosing
* There is limited data on the use of l-ornithine l-aspartate in pediatric patients. Dosing should be individualized and based on clinical judgment and available literature, with caution.
## Dose Adjustments
* No specific dose adjustments are typically required for renal or hepatic impairment, as the drug is intended for patients with liver dysfunction.
## Contraindications
* Severe kidney failure (creatinine > 3 mg/dL).
* Hypersensitivity to l-ornithine l-aspartate or any component of the formulation.
## Adverse Effects
* **Common:** Gastrointestinal disturbances (nausea, vomiting, abdominal pain, diarrhea).
* **Less Common:** Allergic reactions, transient increase in urea, headache, dizziness.
## Key Drug Interactions
* No significant drug interactions are well-documented. However, caution with other medications affecting ammonia levels or kidney function may be warranted.
## Monitoring
* Monitor for signs and symptoms of hepatic encephalopathy.
* Monitor kidney function (creatinine).
* Monitor electrolytes.
## Clinical Pearls
* Oral administration should be taken with meals to minimize gastrointestinal side effects.
* For intravenous administration, slow infusion is recommended.
* The effectiveness of Hepamerz is primarily related to its role in ammonia detoxification and may be more pronounced in patients with impaired urea synthesis.
