# HepaMerz (L-Ornithine L-Aspartate)
## Overview
HepaMerz is a combination of two amino acids, L-ornithine and L-aspartate, which are involved in the urea cycle and protein synthesis. It is used to treat hyperammonemia associated with liver dysfunction.
## Primary Indications
Treatment of overt hepatic encephalopathy and latent hepatic encephalopathy in patients with cirrhosis or other liver diseases.
## Adult Dosing
* **Hepatic Encephalopathy:**
* Oral: 3 to 6 grams three times daily.
* Intravenous (IV): 5 grams twice daily for up to 7 days, followed by oral therapy.
* Dose and frequency may be adjusted based on patient response and ammonia levels. Maximum daily oral dose is typically 18 grams.
## Pediatric Dosing
* Dosing for pediatric patients is not well-established and should be based on institutional protocols and expert consultation.
## Dose Adjustments
* No specific dose adjustments are typically required for renal or hepatic impairment, as the drug's metabolism is primarily independent of liver and kidney function. However, severe hepatic impairment may necessitate closer monitoring.
## Contraindications
