# Hepamerz (l-Ornithine l-Aspartate)
## Overview
Hepamerz is a combination of the amino acids l-ornithine and l-aspartate. It is believed to reduce ammonia levels in the blood by promoting its detoxification in the liver and kidneys.
## Primary Indications
Treatment of latent and manifest hepatic encephalopathy (HE) in patients with liver disease.
## Adult Dosing
* **Oral:** 3 to 6 grams three times daily, taken with or after meals. The total daily dose should not exceed 18 grams.
* **Intravenous:** 5 grams to 10 grams once or twice daily, infused over 30-60 minutes. In acute HE, doses up to 25 grams per day have been used, but the optimal dosage and duration are not well-established.
## Pediatric Dosing
* The use of l-ornithine l-aspartate in pediatric patients is not well-established. Dosing recommendations are not available.
## Dose Adjustments
No specific dose adjustments are typically recommended for renal or hepatic impairment, as the drug is intended for patients with liver dysfunction. However, caution is advised in patients with significant renal impairment due to potential accumulation of the amino acids.
## Contraindications
