## Hepamerz (L-Ornithine L-Aspartate)
### Overview
Hepamerz is a combination of two amino acids, L-ornithine and L-aspartate, used in the management of hyperammonemia associated with liver insufficiency.
### Primary Indications
* Treatment of latent and manifest hepatic encephalopathy.
### Adult Dosing
* **Oral:** 3 to 6 grams three times daily.
* **Intravenous:** Typically 5 grams twice daily as an infusion over 30-60 minutes. Specific infusion rates and total daily doses may vary based on clinical status and local protocols.
### Pediatric Dosing
* Dosing in pediatric patients is not well-established and should be individualized based on clinical response and physician judgment.
### Dose Adjustments
* No specific dose adjustments are typically required for renal or hepatic impairment, as the drug is metabolized by the liver and excreted renally. However, caution is advised in severe renal impairment.
### Contraindications
* Severe renal insufficiency (creatinine clearance < 25 mL/min).
* Hypersensitivity to L-ornithine or L-aspartate.
* Rare hereditary conditions such as fructose intolerance, glucose-galactose malabsorption, or sucrase-isomaltase deficiency should be considered for oral formulations containing sucrose.
### Adverse Effects
* Nausea, vomiting, abdominal discomfort, diarrhea.
* Rare: headache, dizziness, fatigue.
* Skin rash and itching (hypersensitivity).

* No significant drug interactions have been widely reported.
### Monitoring
* Monitor for improvement in symptoms of hepatic encephalopathy (e.g., cognitive function, confusion).
* Monitor electrolytes and renal function, especially in patients with pre-existing conditions or receiving IV therapy.
### Clinical Pearls
* Ornithine and aspartate are thought to help in ammonia detoxification by stimulating the urea cycle and glutamine synthesis, respectively.
* Oral administration may be less effective or slower to act than intravenous administration in acute encephalopathy.
* IV administration should be slow to minimize risk of local irritation or phlebitis.
