# Hepamerz (l-ornithine l-aspartate)
## Overview
Hepamerz is a combination of the amino acids l-ornithine and l-aspartate. It is thought to reduce ammonia levels by enhancing its detoxification in the liver and kidneys.
## Primary Indications
* Treatment of latent and overt hepatic encephalopathy (HE) in patients with cirrhosis.
## Adult Dosing
* **Oral:** Typically 3 sachets (18 g total) per day, divided into 3 doses. Some sources suggest up to 6 sachets (36 g total) per day in severe cases, particularly for overt HE.
* **Intravenous:** Typically 10 g to 30 g per day, administered as an infusion. The exact dose and duration depend on the severity of HE and local protocol. Often given as 5g or 10g per infusion.
## Pediatric Dosing
* Limited data available. Use in pediatric patients is generally not recommended due to lack of established safety and efficacy.
## Dose Adjustments
* No specific dose adjustments are typically required for renal or hepatic impairment, as the drug is intended for patients with liver disease. However, caution is advised in severe renal impairment.
## Contraindications
* Severe renal insufficiency.
* Hypersensitivity to l-ornithine or l-aspartate.
## Adverse Effects
* Gastrointestinal disturbances (nausea, vomiting, diarrhea, abdominal pain) are the most common.
* Rarely, rash or itching.
