# Hepamerz (L-Ornithine L-Aspartate)
## Overview
Hepamerz is a combination of the amino acids L-ornithine and L-aspartate. It is thought to aid in the detoxification of ammonia in the liver by promoting its conversion to urea (ornithine cycle) and glutamine.
## Primary Indications
Treatment of latent and manifest hepatic encephalopathy, particularly in patients with cirrhosis.
## Adult Dosing
* **Oral:** 3-6 grams (equivalent to 1-2 sachets or 1-2 tablets) three times daily, depending on the severity of the condition and patient tolerance. The total daily dose should not exceed 18 grams.
* **Intravenous (IV):** Typically administered as a continuous infusion. Standard dosing is 5 grams twice daily (total 10 grams daily), often diluted in 500 mL of 5% dextrose or saline. Specific infusion rates depend on local protocols and patient response, but often start at 20-25 mL/hour.
## Pediatric Dosing
Dosing in pediatric patients is not well-established. Use with caution and at the discretion of the treating physician, with careful monitoring.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment, as the drug is primarily metabolized in the liver. However, caution is advised in severe renal impairment due to potential accumulation of amino acids.
## Contraindications
* Severe renal insufficiency (creatinine clearance <25 mL/min).
