# Hepamerz (l-Ornithine l-Aspartate)
## Overview
Hepamerz is a combination of the amino acids l-ornithine and l-aspartate, used in the treatment of hyperammonemia associated with liver disease. It is believed to promote ammonia detoxification by stimulating the urea cycle and glutamine synthesis.
## Primary Indications
* Hepatic encephalopathy (in conjunction with standard therapy)
* Hyperammonemia in patients with cirrhosis
## Adult Dosing
* **Intravenous (IV):** 5 g to 10 g (one to two ampules) per 24 hours, infused over 30 to 60 minutes. Dosing and infusion rate may be adjusted based on the patient's response and tolerance.
* **Oral:** Typically 3 to 6 g by mouth per day, divided into 3 doses. The exact oral dosage may vary by formulation and local protocol.
## Pediatric Dosing
* Limited data available for pediatric use. Dosing should be individualized under specialist guidance and based on body weight and clinical response.
## Dose Adjustments
* No specific dose adjustments are routinely recommended for renal or hepatic impairment, as the drug is metabolized by the liver and excreted by the kidneys. However, caution is advised in severe renal impairment due to potential accumulation.
