## Hepamerz (l-Ornithine l-Aspartate)
### Overview
Hepamerz is a combination of the amino acids l-ornithine and l-aspartate. It is thought to help reduce ammonia levels in the body.
### Primary Indications
Treatment of hyperammonemia associated with liver cirrhosis.
### Adult Dosing
* **Intravenous (IV):** 5 grams to 10 grams (as l-ornithine l-aspartate) infused over 30 to 60 minutes, typically 2 to 3 times daily. Dosing can be adjusted based on ammonia levels and patient response.
* **Oral:** 3 grams to 6 grams (as l-ornithine l-aspartate) taken orally, typically 3 times daily.
### Pediatric Dosing
There is **limited data** on pediatric use. Dosing should be determined by a specialist and is highly individualized, often based on weight and ammonia levels.
### Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment, as the drug is metabolized by the liver and its metabolites are excreted renally. However, careful monitoring is still advised.
### Contraindications
* Known hypersensitivity to l-ornithine l-aspartate.
* Severe renal impairment (creatinine clearance < 25 mL/min) due to potential accumulation of metabolites.
### Adverse Effects
* Nausea, vomiting, abdominal pain
* Diarrhea
* Dizziness
* Fatigue
* Skin rash
### Key Drug Interactions
No significant drug interactions are well-established. However, caution may be warranted if co-administered with other medications affecting ammonia metabolism or renal function.
### Monitoring
* Serum ammonia levels
* Renal function (BUN, creatinine)
* Liver function tests
* Electrolytes
* Clinical signs and symptoms of hepatic encephalopathy
### Clinical Pearls
* The oral formulation is often used for maintenance therapy after IV treatment.
* Ensure adequate hydration when administering IV formulations.
* Treatment effectiveness is primarily guided by ammonia level reduction and improvement in clinical symptoms.
