# L-Ornithine L-Aspartate (Hepamerz)
## Overview
L-ornithine L-aspartate (LOLA) is a combination of two amino acids used primarily in the management of hepatic encephalopathy.
## Primary Indications
* Treatment of latent and overt hepatic encephalopathy associated with chronic liver disease.
## Adult Dosing
* **Oral:** 3 to 6 grams three times daily.
* **Intravenous:** Typically 10 to 20 grams per day, infused over 24 hours, for severe cases. Specific dosing regimens may vary based on clinical judgment and local protocols.
## Pediatric Dosing
* Pediatric dosing is not well-established. Use with caution and at the discretion of the treating physician.
## Dose Adjustments
* No specific dose adjustments are typically required for renal or hepatic impairment, as the drug is metabolized in the liver and its components are involved in normal metabolic pathways. However, caution is advised in patients with severe hepatic dysfunction.
## Contraindications
* Hypersensitivity to L-ornithine L-aspartate or any of its components.
* Severe renal failure (creatinine clearance < 25 mL/min) due to potential accumulation of its metabolites.
* Rare metabolic disorders such as argininosuccinic aciduria.
## Adverse Effects
* **Gastrointestinal:** Nausea, vomiting, abdominal pain, diarrhea.
* **Rare:** Allergic reactions.
## Key Drug Interactions
* No significant drug interactions have been clearly established.
## Monitoring
* Clinical signs and symptoms of hepatic encephalopathy.
* Liver function tests.
* Renal function (especially in patients with pre-existing renal disease).
## Clinical Pearls
* LOLA facilitates the detoxification of ammonia in the liver and brain by promoting the urea cycle and glutamine synthesis.
* It can be administered orally or intravenously depending on the severity of hepatic encephalopathy.
* Ensure adequate hydration, especially with intravenous administration.
